Position Overview

An excellent opportunity has emerged within our supportive integrated community team.

Do you possess experience in a rehabilitation setting or as part of a multidisciplinary team?

Join our community therapy team.

We deliver Occupational Therapy and Physiotherapy services to our clients.

Your role will involve managing your own caseload with guidance from qualified therapists. This encompasses falls prevention, mobility evaluations, and rehabilitation following routine procedures, along with the provision of essential equipment and mobility aids. We provide avenues for professional growth supported by our welcoming and forward-thinking team.

Key Responsibilities

This position is full-time, with hours per week, although part-time arrangements may also be considered. If selected for an interview, you will be contacted accordingly.

This role necessitates visiting clients in their residences; therefore, a full, valid driving license and regular access to a vehicle for business purposes are mandatory (unless exempt under the Equality Act 2010).

You will also be required to maintain electronic patient records using SystmOne (training will be provided).

In addition to the base salary, this position includes a High Cost Area Supplement of 5% of the base salary (pro rata where applicable).

Job Responsibilities

The ideal candidate will possess a health-related qualification equivalent to NVQ Level 3 or relevant community experience.

Qualifications & Training

  • Strong general education background
  • NVQ Level III or equivalent knowledge and experience
  • Relevant experience within the NHS

Experience and Knowledge

Essential

  • Understanding of the needs of older individuals and those with acute illnesses
  • Familiarity with functional, cognitive, risk, and mobility aspects of Occupational Therapy and Physiotherapy
  • Knowledge of fundamental assessment components necessary for therapy evaluation
  • Understanding of basic treatment planning and progression
  • Prior clinical and therapy experience working with patients
  • Previous experience or knowledge of health and social care
  • Awareness of available community resources for patient referrals
