We are seeking a highly motivated and organized Care Coordinator to join our friendly and fast-paced homecare team in Plymouth. As a Care Coordinator, you will be the first point of contact for both new and existing service users and care workers, handling initial enquiries and providing excellent customer service.
Key Responsibilities- Be the primary point of contact for service users and care workers, providing support and guidance as needed.
- Effectively handle initial enquiries from prospective service users and their families, ensuring a smooth and efficient onboarding process.
- Roster staff to provide a safe and effective delivery of service to clients, taking into account their individual needs and preferences.
- Liaise with other professionals involved in service users' care, ensuring seamless communication and collaboration.
- Assist with recruiting new care workers to the team, providing support and guidance throughout the recruitment process.
- Monitor care workers and carry out supervisions and appraisals, ensuring they have the necessary skills and training to provide high-quality care.
- Support the effective handling of safeguarding, incidents, and complaints, ensuring that all issues are addressed promptly and professionally.
- Ensure the service is working within the standards expected by the Care Quality Commission (CQC), maintaining a high level of quality and compliance.
- A full UK driving license is essential for this role.
- A good communicator with excellent interpersonal skills, able to build and maintain strong relationships with service users, care workers, and other professionals.
- Flexible and able to cope with changing demands and priorities, remaining calm under pressure.
- Well-organized with excellent multitasking skills, able to manage multiple tasks and responsibilities simultaneously.
- A friendly, confident, and helpful telephone manner, providing excellent customer service at all times.
- A strong focus on teamwork, with the ability to work independently and apply initiative when needed.
- A positive, focused attitude, with a commitment to delivering high-quality care and exceeding targets.
