This role is crucial in facilitating the enhancements and advancements in our ward clinical pharmacy services, working in tandem with the senior clinical team to guarantee the effective implementation and integration of any practice modifications.

Key Responsibilities

In collaboration with the senior leadership team of clinical services, utilize specialized knowledge of clinical pharmacy to spearhead the development, execution, and evaluation of a comprehensive, safe, and high-quality patient-centered clinical pharmacy service for patients and staff at Great Western Hospitals NHS Foundation Trust. Contribute proactively and creatively to meet the evolving demands of the service, ensuring it is delivered in a professional, timely, and efficient manner.

As a Senior Pharmacy Technician specializing in Medicines Optimisation, you will apply your expert clinical knowledge to prioritize the pharmacy needs of patients and wards, effectively supporting the clinical ward/team pharmacist.

With the recent establishment of our integrated front door, this is an exciting opportunity to work collaboratively with pharmacy and Trust-wide colleagues to integrate our clinical pharmacy services into new patient pathways.

Responsibilities of the Role

Advanced Medicines Optimisation & Clinical Responsibilities

  • Advocate for and promote Pharmacy clinical services within your designated division.
  • Lead the development of the Senior Pharmacy Technician role within the division and across the Trust.
  • Operate with minimal supervision to deliver an admission, supply, and discharge service to assigned wards, engaging directly with pharmacists, medical, and nursing staff regarding complex clinical matters and escalating issues as necessary.
  • Upon patient admission, verify that prescribed medications align with patient information, including the Summary Care Record, GP correspondence, and/or Nursing Home details, while meticulously documenting medication histories.
  • Evaluate any medications brought in by patients for suitability and, with patient consent, remove any unnecessary items.
  • Implement clinical prioritization principles and processes in accordance with departmental guidelines, remaining within the scope of your professional capabilities.
  • Order medications not brought into the hospital in accordance with departmental standard operating procedures.
  • Provide patient counseling on medication usage and supply written information as needed.

Qualifications and Experience

Essential Qualifications

  • Registered Pharmacy Technician with the General Pharmaceutical Council.
  • NVQ / QCF Level 3 in Pharmacy Services or equivalent, along with accredited foundational knowledge (BTEC in Pharmaceutical Science or equivalent).
  • Accredited Checking Technician qualification or willingness to pursue accreditation within 12 months.
  • Accredited Medicines Management qualification or willingness to pursue accreditation within 12 months.
  • Accredited in clinical prioritization module or equivalent clinical qualification or willingness to pursue.

Desirable Qualifications

  • Project management qualification.
  • Staff/team management qualification.

Essential Experience

  • Extensive experience in a hospital pharmacy setting, including leadership and team management.
  • Ability to manage personal workload to meet service demands.
  • Experience in a role requiring multi-disciplinary communication.
  • Experience in staff development and training.

Desirable Experience

  • Experience in audit and/or research implementation.
  • Familiarity with national reports on pharmacy service development.
  • Experience in policy and procedure development.
  • Experience in managing change initiatives.
