We are seeking exceptional individuals to join our Review and Support Team within Adult Social Care at Hull City Council. As a Review and Support Officer, you will play a key role in undertaking reviews and reassessments for individuals with long term needs, ensuring they receive the necessary support to maintain their independence and quality of life.
Key Responsibilities- Undertake mental capacity assessments and best interest decisions to enable individuals to stay at home with an appropriate support plan.
- Work in a person centred, solution focused way to identify individuals' strengths and promote independence.
- Arrange support to enable individuals to have a quality of life and remain safe.
- Collaborate with the Review and Support Team to provide ongoing training and development, regular formal supervision, and informal support.
