Glenholme is seeking a dedicated and experienced Lead Care Coordinator to join our team of compassionate healthcare professionals.

In this role, you will play a vital part in providing exceptional support and care to individuals with special needs. You will lead and guide a team of support professionals, ensuring they deliver personalized care plans that meet the unique needs of each client.

Your responsibilities will include:

  • Supervising and mentoring a team of support professionals, providing guidance and support to ensure high-quality care delivery.
  • Collaborating with the Manager to oversee daily operations, ensuring adherence to regulations and standards within our organization.
  • Developing and implementing customized care plans in partnership with healthcare experts, tailoring them to each individual's requirements.
  • Continuously evaluating and enhancing the quality of care provided, identifying areas for improvement and implementing effective solutions.
  • Building strong relationships with clients, their families, and external stakeholders to ensure comprehensive and coordinated care.
  • Providing practical support, including personal care, meal preparation, and medication administration, as needed.
  • Maintaining accurate records and adhering to all relevant regulations, policies, and procedures.
  • Leading internal quality audits to monitor and enhance the standards of care provided.

We are looking for an individual who possesses:

  • Prior leadership experience in a healthcare setting, preferably with individuals with special needs.
  • A compassionate nature with excellent communication and interpersonal skills.
  • Strong leadership and management abilities to inspire and motivate a team of support professionals.
  • A thorough understanding of relevant laws and standards governing the care of individuals with special needs.
  • Flexibility to work varied shifts, including weekends and holidays, to meet the needs of our clients.
  • A genuine commitment to enhancing the lives of individuals with special needs and making a positive difference in their well-being.
