We are seeking an experienced and motivated Clinical Lead to join our Diabetes and Endocrinology team at North Middlesex University Hospital. As a key member of our team, you will be responsible for providing clinical leadership and expertise in the management of patients with diabetes and endocrine disorders.
Main Responsibilities- Provide clinical leadership and expertise in the management of patients with diabetes and endocrine disorders.
- Develop and implement strategies to improve patient outcomes and experience.
- Collaborate with the medical team and other healthcare professionals to ensure seamless service delivery.
- Assess and evaluate patients with complex diabetes and endocrine disorders, and develop individualized care plans.
- Lead and develop standard operational procedures (SOPs) for nurse-led clinics and patient care pathways.
- Coordinate and run nurse-led clinics, and manage and review patients on an urgent basis.
- Develop and implement education programs for patients and carers, and provide support and guidance on self-management strategies.
- Collaborate with the clinical lead and service teams to optimize demand and capacity within the diabetes and endocrinology service.
- Collect, collate, and evaluate information relating to patient referrals and procedural outcomes.
- Work collaboratively with the multi-professional team to develop and implement service improvement initiatives and strategies.
- Network within and outside of the organization to advance the quality of patient care and clinical outcomes.
- Registered Nurse on the NMC register.
- MSc or working towards it.
- Post-registration course in managing patients with diabetes and/or endocrine disease.
- Non-Medical Prescriber.
- Leadership course.
- Experience in insulin pumps and knowledge of national and local audits.
