We are looking for a diabetes support worker/educator to work with the progressive community diabetes team supporting people with both type 1 and type 2 diabetes.

The post holder will provide assessment and support to those recently discharged from hospital or those who are needing support from within the community. The post holder will be given training in XPert structured education and will deliver this to groups alongside colleagues.

The post holder will work as part of the Community Diabetes Team specialist diabetes team, but also supporting community nursing teams as an autonomous practitioner in the provision of evidence based and high-quality diabetes care.

The post holder will participate in the delivery of care interventions and undertake delegated tasks such as blood pressure, blood glucose testing, provide assistance primarily in patient homes, as well as the delivery of structured Type 2 education.

As a member of the diabetes and wider community teams, it is important that you are able to prioritise and contribute to the delivery of high standards of care to patients, families and their carers in a safe and supportive environment under the direction of a registered practitioner. As part of the wider team you will also be asked to assist in developing and maintaining services provided to patients and contribute to the safe, efficient and smooth running of the clinical service.

Part of the role will involve rotation to support community nursing teams in the diabetes management of patients on their caseloads, including administration of insulin

To work with the multidisciplinary teams (specialist diabetes team and community nursing teams) to provide a standard of care to patients with Type I & Type 2 diabetes.

Undertaking ‘information gathering’ visits within peoples homes, as well as undertaking simple nursing procedures under the supervision and direction of the Diabetes Specialist Nurse/Community Nurses:

- Blood glucose and HbA1c testing (phelebotomy)
- Blood pressure measurements
- Height/weight/waist circumference
- Ensuring those discharged from hospital or in-patient unit have the correct equipment and medication that is recorded on the discharge summary
- Use of Freestyle Libre and educating patients on this as needed
- Insulin administration to those on community nursing caseloads

In accordance with delegated tasks, organise own workload, contacting patients by telephoning prior to their appointments

Maintain accurate, legible and up-to-date records on all patients in accordance with Diabetes and community nursing services, as well as organisation policies and procedures.

Work with Community Nursing and Neighbourhood Nursing teams to support those patients with diabetes who may not be under the specialist diabetes service but need support with review and insulin administration

Deliver and co-deliver structured patient education for Type 2 diabetes (currently XPert), and to assist in the evaluation and audit of this structured education

Participate in staff development and appraisal and to agree and review a Personal Development Plan for ongoing development, undertake training as appropriate.
