Ambition Community Healthcare are a leading provider of complex healthcare supporting adults and children living with complex and life limiting conditions across Norfolk and Suffolk.
Founded in 2019, we have seen exceptional growth over the last 3 years and we have an exciting new role available within our office based team.
We are looking for a new Care Co-ordinator to join the team to support the Registered Manager in the daily operations of care co-ordination and case management.
- **What does the role entail?**_
- Managing the rostering software system ensuring all packages of care are fully staffed and receive rotas in a timely manner
- Ensuring adequate cover for holiday and sickness
- Liaising with Support Workers daily to ensure they are aware of any changes to rotas and updating software
- Liaising with new staff and clients to arrange meet and greets and shadow shifts
- Liaising with the nursing team to ensure delivery of training for packages of care in line withy client need
- Completing wellbeing calls to all of our staff as part of their induction process
- Conducting staff supervisions
- Assisting with on-call out of hours on a rotational basis
**Job Types**: Full-time, Permanent
Pay: £22,500.00-£25,500.00 per year
