**About The Role**
**Location**: Community based, with some home working and office workingin Bristol to cover access point for the service on a rota basis.
**Contract**: Fixed Term Contract for 12 Months.
We are looking for an enthusiastic, motivated, and highly professional individual who would like to be part of Bristol Dementia Wellbeing Service. This service in an innovative service working in partnership with NHS in Bristol where you can make a real differenceto the lives of people affected by dementia.
The Bristol Dementia Wellbeing Service provides responsive and individualised support to people with dementia and their carers within their community. Our dedicated team provide information, advice and signposting; face-to-face, telephone and online to supportpeople to live well with dementia and prepare for the future.
You will be part of a highly motivated and supportive team, sharing responsibility of some admin tasks, including office based access point telephone cover as part of a rota. Build relationships with a range of local contacts, networking with health and careprofessionals and organisations who work in the field of dementia so that you can advise and support people affected by dementia to access services and gain information that will help them to live as well as possible within their community.
**About you**:

- You will have an understanding of the needs of people with dementia and those who care for them, and be able to demonstrate a person centred approach.
You will need a broad understanding of statutory and voluntary services, and how they can support people affected by dementia
- You will be comfortable connecting with others and talking to a wide range of other health care professionals and related statutory and voluntary organisations.
- You will have good communication and organisational skills.
- You will need the ability to travel independently around Bristol, be a team player, supporting colleagues across Bristol, and be able to ask for help yourself when needed.
