The purpose of the role is to assess and establish patient need and urgency of all new external referrals into Ashgate Hospice, in order for referrals to be received by the right service at the right time. The post holder will work closely with the lead clinical assessor to allocate and contact patients and referrers to establish priority and provide a timely and efficient response to all referrals and record outcomes. They will also respond to advice calls in a timely manner and offer quality, professional advice and support to patients, families and health care professionals, and be able to signpost them as appropriate to other relevant services. The post holder will have excellent communication and documentation skills and be a motivated and flexible worker.

The post holder will work closely with the current SinglePoint team and wider team members within the organisation, to continue to develop the SinglePoint of Access service.
