This is an excellent opportunity to join our Community Specialist Palliative Care Therapy team for anyone who has an interest in supporting and improving the quality of life for patients with life limiting illnesses. Whether you are an experienced Band 4 wanting a change or a Therapy assistant wanting to develop their skills working in a specialist area.

The post is part of new funding to develop the specialist palliative care team working alongside the physiotherapist and Occupational Therapists as well as the nurses, psychologist and medical consultant in the team to provide integrated care.

The post holder will be expected to have experience working as an assistant with OT or Physio in the NHS and an understanding of palliative care.

As well as completing joint visits with therapists the post holder will be expected to hold their own caseload, undertaking independent assessments and interventions to provide exercise and mobility programs, ADL practice, education and equipment provision with the aim of improving quality of life and function for each patient.

The ability to travel independently is an essential part of this post as you will work across Middlesbrough, Redcar & Cleveland community providing a service within patients own homes.

As part of the Gold Standards Framework for people nearing the end of their life as part of the team you will work closely with the acute palliative Therapy team based at James Cook University Hospital as well as community therapists and Teesside Hospice to provide a seamless pathway for the palliative patient journey and improved patient outcomes.

It comprises two acute hospitals, James Cook University Hospital and Friarage Hospital Northallerton and a number of community hospitals and community services. This is a unique partnership between Health and Social care in South Tees with a commitment to working together, improving the quality of our services and getting rid of existing barriers between health and social services.

This post offers development in clinical and leadership skills and the post holder will be supported to attend relevant courses. We have an established and comprehensive training program and our staff are encouraged and supported in their CPD through workshops, clinical supervision and the appraisal process.
