An exciting opportunity has arisen for a Band 7 Clinical Specialist - Nurse (fixed term/secondment - 12 months) to work into Stockton Primary Care Network (PCN) to support the needs of the local population living with mental health difficulties. This post supports people in primary care settings and working in a collaborative and multiagency way to meet the needs of patients with a range of mental health needs.
The post will sit within the GP practices and will offer assessment, diagnosis and treatment and review for both working age adults and older people. The role will be offering specialist mental health clinical care. The role will also offer networking expertise and navigation to a range of specialist mental health supports to ensure each person once assessed receives the best possible care in the right place at the right time by the right supporting service.
Candidates should have extensive clinical knowledge and experience across a range of mental health settings. Candidates will need to hold or be willing to work towards a prescribing qualification and be able to offer diagnosis and treatment plans across all mental health conditions and presentations. Experience in leading meetings, presentation skills and offering training and education to professionals across a range of specialities will be essential.
Work with patients who have a severe mental illness and/or complex emotional needs who do not meet the criteria for secondary care services and are too complex for IAPT services to support shared decision-making about self-management; and provide brief evidence-based psychological interventions, where qualified to do so, where the intervention can be safely offered outside of a multi-disciplinary mental health team
PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support
Provide a consultation, advice, triage and liaison function, supported by the local community mental health provider through robust clinical governance structures to maintain quality and safety, including supervision where appropriate
Provide clinical leadership to other ARRS mental health practitioner roles, such as Mental Health and Wellbeing Coaches
Support will be given to you by both the PCN and other staff that work in the Tees Valley PCN Service within TEWV. Other members of the team are an Associate Nurse Consultant, Team Manager, Advanced Mental Health Practitioners, Senior Mental Health Practitioner and Mental Health and Wellbeing Coaches.
Current Professional Registration with Nursing and Midwifery Council - RMN / RNLD/ RGN (RNSC - CAMHS Only
Postgraduate diploma in a subject relevant to the area Or be willing to work towards and achieve within 2 years and
Within the first year of being in post hold a recognised qualification in a governed therapy/advanced clinical qualification e.g Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy /NMP (Independent Prescriber)
And have Equivalent clinical experience to that of a Post Grad Diploma/master's level such as working as a band 6 (or above) in relevant clinical area with significant experience (see below under experience), together with evidence of relevant CPD
Significant experience working with patients with complex needs in the specialist field.
Significant experience working with patients who display risk behaviours.
Clinical risk assessment and management and its application in practice.
Quality improvement activities.
Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally
Understanding and working knowledge of the legal framework and statutory obligations under the Mental Health Act, Mental Capacity Act
Detailed understanding of Safeguarding and its application in practice.
Clinical Risk Assessment and Management and its application in practice.
Working knowledge of clinical medicines management.
Understanding of psychological models of care and treatment (depending on specialist area).
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
