**Salary: £23,445-£28,019 per annum. Please note, starting salaries are usually set at the bottom of a scale**

Martin House has been caring for babies, children and young people with life-limiting conditions from West Yorkshire, North Yorkshire and the Humber for more than 30 years. Every year we care for more than 420 families at our hospice in Boston Spa, in hospitals and in their own homes. We also support more than 150 bereaved families each year.

The key purpose of this exciting new role is to work alongside the Head of Family Support and Transition to support young people and their families as they transition from Martin House Hospice to Whitby Lodge and as they transition from children to adult palliative care services.

This role will also provide emotional, social and practical support to children & young people with palliative care needs and their families, personalising to their needs. This postholder will be part of a compassionate and creative team delivering a high-quality family support services within the region.

The Family Support team includes therapies, bereavement, spirituality, psychology, transition and wellbeing elements of care.
- Be dedicated, compassionate, work in collaboration, be open, flexible, responsive and caring to deliver high standards in everything you do.
- Practice the Martin House values including being child and family focused, listening and valuing others, working holistically and being dedicated to care.
- Want to work in a well-established, award-winning hospice, working as part of the multidisciplinary team to support the children, young people and their families from the time of their referral through to end of life care.
- Have significant experience of working in a support role within a service for Children, Young People and their families. This will preferably be a service for children with complex or palliative care needs.
- Be good at building rapport and making people feel at ease.
- Have good IT skills and ability to record care accurately and contemporaneously.
