Clinical Lead for Complex Discharges and Flow

You will develop your critical thinking and decision-making skills, weighing up clinical need against resources, safety and patient choice. You will need a sound knowledge of the Mental Capacity Act and Best Interest Decisions and be able to think laterally to meet patient need. You will provide leadership and support to the complex case part of the team, specifically within the area of flow and complex discharges. You will be responsible for supporting flow of individuals through the pathway 3 process, including enhanced care reviews and Continuing Healthcare (CHC) assessment, supporting discharges of our most complex individuals and leading our neurorehabilitation process.
