As our Clinical Lead you will coordinate and facilitate the safe and effective discharge of patients from hospital, supporting the proactive reduction in delayed transfers of care. Your role will see you providing assessments for intermediate care services and working proactively within integrated health and social care teams to promote channels of communication both within the team, CCG and wider community.
