**Band 6 Occupational Therapist**

**Community Mental Health,Based in Barnet, North London**

**Full or part-time hours available**
- **An exciting opportunity has arisen to work within the secondary adult mental health community services at a time of innovation and service development.**_

This role will be involved as part of the wider team, in helping to adapt the existing service within the new transformation process. This process is in response to the NHS 5 year plan to develop mental health services round the Primary Care Networks andprovides new opportunities which include developing partnership working with the voluntary sector, working more closely with GP practices and developing more innovative, community based and holistic ways of working.

**Responsibilties include**:

- Carry overall responsibility for the management of a caseload, acting as a keyworker and using the Dialog + care planning tool.
- Deliver effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies.
- Design individual programmes of care collaboratively with patients to maximize self-management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user needs.
- Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psycho-education.
- Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care.
- Facilitate vocational rehabilitation and those aspects of mental health recovery concerning occupation.
- Maintain clear, comprehensive and contemporaneous clinical records on all patients in response to personally generated clinical observations, in accordance with professional standards and Trust policies..
