**About the role**
The postholder will be working across the Localities and Primary Care Team and the Public Mental Health Team in Public Health, engaging with communities in Leeds with the highest risk of poorer health outcomes. Initial priorities will be:

- Using public health intelligence to survey and assess a population’s health and wellbeing
Supporting community-based research in priority inner city wardsz.
- Supporting consultation in communities that will support the development of local Health Needs Assessments.
- Building understanding around access, experience and outcomes
- Working with a range of different groups and communities to gather insight to understand what works in accessing support and the barriers to access.
- Provide key recommendations and principles on what would make services more accessible and culturally appropriate.
- Support public health to test the recommendations and principles and evaluate the impact.
- Audit information and resources published by public health and broader partners to ensure they are accessible, easy read and clear.
- Public Health Project Support
- Providing project support across Localities and Primary Care and the Public Mental Health teams in response to identified needs.

**Job purpose**
To contribute and support the implementation of specialist local health improvement programmes ensuring the implementation of Public Health policies, strategies and work programmes which includes developing and maintaining public health information and support structures.

The post holder will have specific responsibility for developing and implementing a range of initiatives in relation to the full spectrum of health and wellbeing.

The post aims to actively work to address health inequalities within Leeds.

The post holder will also be required to facilitate and support multi-agency public health group work; community based health needs assessment and provide public health related training to a range of staff and community groups.

**Responsibilities**
- Support the continuous development, implementation and evaluation of public health work.
- Facilitate the implementation of health and wellbeing programmes.
- Support relationship building with public health providers to enable contract monitoring.
- Develop, continually review and update action plans.
- Support delivery of programmes to raise awareness and increase early detection in relation to the health and wellbeing of people of the specified locality or topic area.
