Clinical Specialist Practitioner for

Post holder will play a key role in the development & delivery of the Rapid Diagnostics Service (RDS) throughout the Trust. To give advice & implement patient care in an area which requires specific clinical knowledge, skills & experience. To work alongside other clinical colleagues to ensure quality of care to the patients is maximised. The post holder will be part of the Gastroenterology CCNS team.
The CNS will be responsible for developing & delivering appropriate diagnostic & supportive pathways for both patients & Health Care Professionals, utilising & developing appropriate clinical services to provide an accessible & seamless pathway
**The post holder will**:
- Support seamless care of patients with symptoms that could be due to cancer
- Provide a keyworker role and point of access for support and advice
- Support appropriate and timely investigation of patients
- Provide knowledge and advice for both primary and secondary care in regard to pathways/protocols and appropriate onward referrals
- Develop key relationships with primary and secondary care services (oncology, ED, acute medicine, radiology, endoscopy, and pathology to ensure quality of care to the patients is maximised)
- Support development of Regional and local clinical guidelines & education programmes
- Support ambulatory care for patients on the pathway, preventing hospital admissions
- Be an integral part of the RDS multidisciplinary team (MDT)
- Plans and develops clinical protocols for specialist area.
- Works autonomously to manage his/her own caseload of patients whilst working as part of a multidisciplinary team.
- Enables patient choice and involvement and initiates appropriate action.
- Coordinates and develops patient care pathway and liaises across organisational boundaries.
- Plans specialised programmes of care and provides highly specialised advice to patients, relatives and carers.
- Ensures each patient is placed in the correct treatment pathway at the appropriate time and necessary investigations and treatments are organised.
- Provides clinical advice and support to healthcare professionals within and across organisational boundaries.
- Provides clinical advice in accordance with current evidence-based practice.
- Undertakes nurse/practitioner -led clinics, participates in consultant led clinics and multidisciplinary team meetings.
- Participates in case conferences in relation to future management or discharge arrangements.
- Ensures patients and their families are given appropriate oral and written information regarding planned care.
- Maintains accurate patient records and ensures all relevant information is documented in the patient’s medical and nursing records.
- Work as an independent nonmedical prescriber (adhering to Trust policy) with the ability to initiate and adjust medication within your field of expertise. (this can be deleted if not required service need)
