Band 8a Care Home Support Team Lead

To provide strong operational leadership to the multi-disciplinary Care Home Support Team as well as demonstrate strong clinical skills. The role is split 50/50% between managerial and clinical responsibilities.

The CHST has been developed in partnership with the GP Confederation., who are the Lead Provider of the Service. As the lead you will be expected to put measures in place to ensure high standards of care and to respond to both national and local drivers to identify development opportunities, build system wide relationships and ensure the delivery of both operational and performance targets.

A key part of the role is relationship building. You will be expected to represent the Care Home Support Team across the wider system and engage with partner organisations to develop and enhance service delivery. Attending both internal and with external meetings with partners, contributing to the NWL ICS. This includes representation as part of End of Life and Discharge workstreams.

The post holder will have oversight of part of the budget and be expected to produce various reports and audits which requires knowledge of IT systems and working with data.

You will hold your own caseload acting as a role model and supporting a number of Care Home Facilities across Hillingdon, having overall oversight of all.
- The post holder will be responsible for overseeing all operational aspects of the Care Home Support Team (CHST) as well as providing professional Line management for Care Home Matrons and Allied Health Care Professionals within it
- To have overall responsibility for the effective delivery of the CHST working in partnership with the identified stake holders.
- To ensure the both the Care Home Support Service and GP element of the service is working appropriately and meeting specification requirements
- To provide clinical leadership and management support for the service working in collaboration with the Confederation Clinical Lead
- To undertake highly skilled and complex care activities directly related to the assessment, planning and evaluation of patient care.
- To make skilled clinical decisions and communicate the rationale clearly.
- To ensure that care delivered at end of life is culturally sensitive and addresses issues of diversity
- To coordinate an effective MDT and ensure effective specialist contribution to other teams MDTs.
- To provide line management for the staff working within the service ensuring that individuals are appraised and supported in line with CNWL policy, including regular 1-1 support.
- disciplinary team of Service Leads. Accessing the benefits of two organisations, the GP Confederation and supported by CNWL Management structure, guides and policies.

Service Leads are encouraged to be innovative and committed to service improvements including playing an active role in change management.

You will be responsible for ensuring all aspects of the day to day operational delivery of the Care Home Support Team. You will be expected to be visible, providing clinical support when necessary. You will be working alongside senior management, commissioners, the CCG and multi-agency providers, families and carers.

You will be expected to work closely with managers from partner organisations to line manage the team and meet service targets. You will need to be forward thinking and able to provide integrated leadership. This role requires excellent leadership, organisational, time management and communication skills. This role provides an opportunity to play an integral role in shaping community services for the future.
