**Job summary**:
The post holder will deliver an essential role within the Urology Service through developing and utilising effective tracking processes to ensure that patients receive timely appointments and treatments in line with their clinical management plan. The tracking of patients ensures that no one should 'slip through the net' at any point in their pathway and that processes are in place to ensure robust monitoring of all patients.

The Failsafe Officer is responsible for ensuring that all patients receive review and treatment from the right person, in the right place, within their specified clinical timeframe.

The Failsafe Officer will monitor all Urology patients who have been added to a waiting list for treatment and ensure each patient has their intended date for treatment documented and appointments are booked, as appropriate. The Failsafe Officer will achieve this by:

- Identifying, investigating, reporting and escalating all overdue treatments
- Booking TCIs and pre-op assessments
- Auditing, evaluating and reporting on DNAs and cancellations
- Identifying gaps, inconsistencies, errors and/or unwarranted variation in clinical risk stratification or prioritisation of TCI's, ensuring treatments are completed, with outcomes recorded and monitored.
- Tracking patients to ensure they are not lost on waiting lists.

It is important the applicant is confident working with IT systems and has experience of working with excel, undertaking data analysis and creating reports

**Main duties, tasks & skills required**:
1.Ensure that all patients are booked for surgical interventions and are arranged appropriately in line with current national and local targets.

2.Maintain an accurate list of all identified high risk or cancer patients and ensure that they are booked for pre-assessment and subsequent surgical interventions arranged appropriately in line with current national and local targets.

3.Liaising with consultants and inpatient booking staff, ensuring that all pre-assessment appointments are scheduled appropriately.

4.Ensure that patients who Did Not Attend their pre-assessment clinic are contacted within appropriate timescale and reschedule the appointment in line with the Trust's and the department's DNA & Access policies.

5.Ensure that the Urology PTL is validated and accurate for all patients including those under diagnostic appointments.

6.Be a key point of contact for patients with concerns regarding their delayed admission.

7.Regularly meet with consultants to review the waiting list tracker.

8.To work within the Urology department to develop failsafe systems to ensure patients receive timely appointments and treatment by:
a) Tracking patients through the system and auditing their care, ensuring those who require treatment receive it using local and national guidelines.

b) Where warranted booking of patient appointments and treatments into a range of clinics.
