For the right person this is a rare opportunity to work within a small but dynamic home therapies team. The role requires someone able to support the existing & expanding home therapies with skills & experience in haemodialysis. The role is varied & involves working with the multidisciplinary team to provide the highest quality innovative care when supporting patients to undertake their treatment in their own home. Working in collaboration with our Regional Renal network our strategy is to increase our home haemodialysis population to equal 20% of our total dialysis population.
We are looking for a patient focused, enthusiastic person to join the multi-disciplinary team.
Access to a car and possession of a clean driving license is essential to the post. You will need to be prepared to work flexible hours at times to ensure the requirements of the service are covered.
Previous renal experience with haemodialysis skills essential. The Trust has an excellent educational programme encouraging personal development.
You must be flexible in your approach to work; this includes travelling offsite and visiting patients at home.
With support from the renal specialist nurses you will be involved in training patients to perform their own treatments. These patients then require follow up at home, taking blood specimens & helping troubleshoot & resolve problems. You will be proficient in needling & familiar with the use of a Sonosite to assist with assessing access.
The role requires good communication & interpersonal skills to enable good professional relationships imparting health education & sound clinical practice clearly & nonjudgmentally.
DCH is the Regional Renal Unit for Dorset & South Somerset. The service covers a population of 1.49 million patients at four acute hospitals including Yeovil, Poole and Bournemouth. We have satellite units in Poole Yeovil & Bournemouth with plans for another.
Prince of Wales is our 15 bedded inpatient ward receiving referrals from the other local acute Trusts Inpatient support is often required for more than 330 haemodialysis patients, 26 peritoneal dialysis patients, 443 transplant patients, 300 low clearance patients & 3000 chronic renal failure patients.
Here at Dorset County Hospital our renal unit cares for patients at all stages of their condition and provides the supportive therapies, care and treatments required from an outpatient to an acute care setting. The team offers a full range of specialist services in order to support patients in a timely, safe supported and effective journey throughout the health care system. We offer pre dialysis care through specialist teams as well as ongoing programmes of care such as kidney transplantation, haemodialysis and peritoneal dialysis. We aim to provide patients with all the information they need to make informed choices.
