The post holder will be the lead practitioner working with vulnerable young people. The focus of the post will be to work with young people using and affected by substance misuse. This involves working with the young people and being sensitive to the "children first'' environment. The post holder must engage with young people using, affected by and at risk of developing problematic drug use, and in doing so, also provide support to parents, teachers and other relevant practitioners. The development and sharing of skills for front line workers is vital for a holistic and consistent approach across service providers.
- To provide outreach substance misuse interventions to young people aged 10-21 years in the London Borough of Islington. The post holder will achieve this by:
- Managing a caseload of young people, acting as substance misuse care coordinator/lead practitioner, to identify needs and deliver care and/or treatment plans, referring to other health and social care agencies as required
- Providing a service to referred young people which will include comprehensive assessments, one to one sessions, group work and referrals to other services
- Delivery of advice, information and training to young people, their families and other professionals, including work in Islington schools

**OVERALL REQUIREMENTS**:
1. Work proactively to provide outreach substance misuse services to vulnerable young people in the borough (focussing on young women and girls) and work as part of a team of young people's substance misuse workers.
2. To provide training, information, advice and guidance to the parents/carers and staff who work with vulnerable young people, ensuring that the needs of young drug users are recognised and understood
3. To undertake accurate and timely assessments using the range of agreed assessment tools, with young people and their families, and other professionals where appropriate, to identify substance related needs and develop appropriate care and/or treatment plans.
4. Hold a case load of up to 20 individual young people at any given time (tier 2, 3 and group work sessions)
5. Keep accurate and up to date records of work undertaken, care plans, decisions made, actions taken and outcomes achieved, providing case studies and reports as required by managers.
6. In conjunction with the Lead Professional and the Stronger Families Team, to identify and refer appropriate families for specialist family interventions.
7. To work directly with young people on an individual or group work basis in order to offer harm minimization interventions, advice, support, and treatment to young people around substance misuse.
8. To recognise the professional duty of care to protect where clients present a risk to themselves or others, and to comply with statutory requirements regarding child protection including the Children Act 1989 and Hidden Harm guidelines.
