Overview:
**Change Grow Live** are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.

We have an exciting opportunity for an **Aftercare Coordinator** **who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. With this role you’ll be working as part of the Renew aftercare team, working across all three of our sites. This role will involve carrying a caseload and will primarily be working with our Aftercare Cohort, supporting those who have become abstinent from substances.

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

**Salary**:£27,449.53 - £31,529.41 per year, pro rata

**Hours**: 22.5 per week

**Contract type**: Permanent

**Responsibilities**:
**About the role**:

- To provide recovery coaching using a broad range of psychosocial interventions including group facilitation inclusive of SMART.
- Effectively assess, plan, monitor and review all stages of client’s aftercare journey using designated tools within agreed time scales.
- Coordinate, Contribute & participate towards multi-disciplinary meetings.
- Provide assertive outreach to service users homes and support to appointments ensuring social inclusion and maximising engagement.
- To promote and support individuals into both abstinence based and medically assisted forms of recovery.

**About you**:

- Relevant experience of working within substance misuse (including alcohol) services with a clear understanding of the need for and ability to deliver quality services.
- A commitment to multi-agency working and establishing partnerships with other professions to achieve good outcomes for the service user group, e.g. mental health services, criminal justice agencies, community groups, etc.
- Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
- Good working knowledge of mental health interventions, services and good practice.
- Ability to manage change successfully in a way that prioritises the needs of service users.
- Ability to liaise and work in partnership with a wide range of professionals and agencies.
- Ability to communicate complex information to individuals and groups in a way that is easily understood.
- Effective interpersonal skills with the ability to engage successfully with internal and external stakeholders at all levels.
