Trainee Advanced Clinical Practitioner

The team of ACP's will be the clinical leaders for the virtual wards, overseeing patient admissions, managing the nursing team to ensure all patients receive the care required. The ACP's will be the liaison point with the consultants and acute clinical teams for patient escalation and progression. Between 8pm and 8am the ACP will be the point of contact for patients on the virtual ward and will use the agreed SOP's to escalate care to the relevant acute team when needed.
This is a training post to become an Advanced Care Practitioner (ACP). The successful applicant will work as part of the ACP team during their training period with the support and oversight of consultants and experienced ACP's. The team of ACP's will be the clinical leaders for the virtual wards, overseeing patient admissions, managing the nursing team to ensure all patients receive the care required.
The trainee, working under supervision, will develop advanced knowledge and skill to meet the capabilities presented in the multi-professional framework for advanced clinical practice in England. It is expected that the trainee’s level of autonomy will increase to the point of independent practice within the specialty.
- Lead daily virtual ward rounds when on shift.
- Use advanced knowledge and skills to undertake comprehensive clinical assessments, make decisions based on clinical reasoning and initiate, evaluate and modify a range of interventions
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. The ACP's will be the liaison point with the consultants and acute clinical teams for patient escalation and progression. Between 8pm and 8am the ACP will be the point of contact for patients on the virtual ward and will use the agreed Standard Operating Procedures (SOP's) to escalate care to the relevant acute team when needed.
- The ACP's will provide professional senior clinical leadership and support within the specialty multi professional team and across service boundaries
- Lead clinical audits and critically engage in research activity using the results to underpin own practice, inform the practice of others and make organisational recommendations
- The role requires an expert clinician who is a resource for the trainee ACP's
- To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.
