Co-ordinate the duties and work plan for care workers
- Assist the Registered Manager with the recruitment of new care workers and their induction
- Ensure that the care worker rotas are organised and sufficiently well planned, taking into account travel time and routes
- Liaise with the Registered Manager to arrange shadow shifts for new care workers
- Plan and allocate care assignments to appropriate care workers, with due regard for the particular needs of the individual and the skills and attributes of the worker
- Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care
- Dealing with queries from clients and families calmly and professionally.
- Carry out on-call coordinating and response duties outside regular office hours as agreed
- Support the Registered Manager with covering care visits for any last-minute schedule changes as and when required
- Take responsibility for new referrals from private clients, social services and the ICB, ensuring complete information is received and appropriate assessments and care planning are undertaken
- Support the Registered Manager with completing care plan reviews and risk assessments.
- Maintenance of electronic and handwritten records

**Person Specification**:

- At least one year's experience in a similar role
- Excellent time management skills
- Achieved at least Level 3 in Health and Social Care or willingness to work towards it
- Must be a driver and have access to a car (essential)
- Have the ability to work collaboratively with other team members
- Should be prepared to undertake other training that would benefit you in the role
- Must be able to demonstrate good communication and interpersonal skills
- Be self-motivated, responsible and able to act on your initiative
- Have previous experience in Domiciliary Care
- Must be organised and flexible
- Flexibility with time to cover shifts or sickness
- Previous experience with rota planning and allocating care staff
- Ability to work under pressure and to deadlines
