Northern Healthcare is an independent provider of supported living services for individuals who require support due to a mental health diagnosis, learning disability or autism.

As a Referrals Assistant, you will play a vital role in ensuring the smooth and efficient coordination of referrals to our supported living programs, contributing to the well-being and recovery of our Residents.

**Qualifications and Skills**:

- Previous experience working in health and social care administration.
- Excellent organisational skills with the ability to manage multiple priorities and meet deadlines.
- Strong communication skills, both written and verbal, with the ability to engage and build rapport with diverse individuals and stakeholders.
- Empathy, compassion, and a genuine commitment to improving the lives of individuals with mental health challenges.
- Proficiency in using computer systems and software for data management and reporting.
- Knowledge of local mental health resources, services, and referral pathways is an asset.

Where you’ll be working

You will be based out of Head Office located in Eccles, M30

Working Hours

The role is a full-time position working 37.5 hours per week. This role requires a level of flexibility

**Key Responsibilities**:

- Assist the Referrals Co-ordinator as the primary point of contact for incoming referrals from various sources, including mental health professionals, social workers, hospitals, and community organisations.
- Collaborate closely with our clinical team to gather and review relevant information, ensuring a comprehensive understanding of each client's mental health condition and support requirements.
- Coordinate and schedule assessments, interviews, and tours for potential clients and their families.
- Maintain accurate and up-to-date records of referrals, including client information, progress, and outcomes.
- assist the Referrals Co-ordinator in providing regular updates and reports to internal stakeholders on the status of referrals, highlighting any challenges or areas for improvement.
- Build and maintain strong relationships with external stakeholders to ensure a streamlined referral process and facilitate effective communication.
- Stay informed about industry trends, best practices, and regulatory requirements related to supported living and mental health services.
