Vacancy Description

Are you passionate about making a positive difference in people's lives? Do you have experience in providing care and support to vulnerable individuals?

If so, we have an exciting opportunity for you to join our dedicated Adult Social Care Service at West Berkshire Council as a Care Practitioner. Reporting to the Support Outreach Worker, you will play a crucial role in delivering high-quality care and support to adults with a range of needs.

Your main responsibilities will include:

- Assisting individuals with personal care, promoting independence and maintaining dignity
- Supporting individuals in their daily activities, ensuring their safety and well-being - Building positive relationships with service users, their families, and other professionals
- Collaborating with the wider care team to develop and implement care plans
- Maintaining accurate records and documentation

To excel in this role, you should have:

- Previous experience in a similar care role, ideally within a social care setting
- A compassionate and person-centered approach to care, with a genuine desire to make a difference
- Excellent communication and interpersonal skills, with the ability to build rapport with a diverse range of individuals
- Strong organisational and time management abilities, with the flexibility to adapt to changing priorities
- A relevant care qualification or willingness to work towards one
