We are looking for registered Health Visitors to join our diverse team of health visitors who make a difference every day to the lives of families in Lambeth and Southwark

Working within either Lambeth or Southwark, you will be based in our main office, within community clinics and agile working. You will work within the Health Visiting Service, working closely with Community Staff Nurses, Child Development Support Workers and other skill mix staff. Together, you will be providing nursing support and care to local families.

**Part of the main duties involves**:

- To organise the delivery of Health Visiting services to a defined population, including home visiting and provision at other sites
- Undertake health needs assessments at home and virtually if required with Children and families and working in partnership with other agencies/services, to implement appropriate individual programmes of care
- Undertake family centred enhanced support with families with complex health and social needs to improve outcomes for children and families

Main responsibilities
- To adhere to the NMC Code of Professional Conduct and be conversant with current professional advice and
- To maintain own level of clinical competence by reading relevant literature and taking opportunities for personal and professional development
- To use evidence based practice to develop and maintain a high quality standard of health visiting
- To participate in audit/ research as required by the health visiting and the Trust
- To organise the delivery of Health Visiting services to a defined population, including home visiting and provision at other sites
- Undertake health needs assessments at home and virtual if required with clients and working in partnership with other agencies/services, to implement appropriate individual programmes of care
- Undertake family centred enhanced support with families with complex health and social needs to improve outcomes for children and families.
