Job Overview:
Mitcham Family Practice is looking to recruit a Physician Associate, who is registered on the PAMVR and is enthusiastic about primary care. The successful applicant will provide safe, and evidence based clinical care to patients of Mitcham Family Practice, supporting others in the clinical team. This will include history taking, clinical assessment, diagnosis, treatment, and evaluation of their care under the supervision of a named general practitioner. The post holder will demonstrate critical thinking in the decision-making process, and they will work with the entire clinical team to meet the needs of patients. The post holder will work with the supervision and support of GPs and others as appropriate, implementing agreed management plans and following approved protocols where necessary.

**Responsibilities**:

- Provide first point of contact for patients presenting with undifferentiated, un-diagnosed problems, utilising history taking, physical examination, problem-solving and clinical decision-making skills to establish a working diagnosis and management plan working in partnership with patients and ensuring appropriate follow-up where required.
- Undertake as agreed; surgery-based consultations for emergency or routine problems including management of long-term conditions.
- To undertake clinical sessions consisting of either telephone triage appointments, face to face appointments or both and participate in duty rotas.
- Instigate necessary invasive and non-invasive diagnostic pathology tests or investigations and interpret findings/reports within the scope of a PAs practice. Discuss the result and implications of laboratory investigations with patients and review of clinical letters.
- To act appropriately upon alerts including QOF, medicines management, local enhanced services and reminders during a consultation and contribute to the practice quality targets to consistently achieve high standards of safe, evidence-based, cost-effective patient care and service delivery
- To support the duty doctor by undertaking telephone triage, and provide follow up consultations for patients as required
- Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support with an emphasis on prevention and self-care
- Refer patients directly to other services or agencies using appropriate referral pathways
- Develop consultation skills to enable shared patient practitioner decision
- Justify the choice of medication and be able to understand the impact of comorbidities and other medications, poly-pharmacy on agent choice and prognosis
- Provide medical support as required to patients referred by nursing staff
- Adhere to the practice prescribing formulary, local and national guideline
- Ensure the practice EPS (electronic prescription rate, paperless prescription generation) is maximised
- Under supervision of the GPs, make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the practice
- Ensure safe handover of care within and outside the practice as appropriate
- Record clear and contemporaneous consultation notes on the EMIS system and to agreed standards
- Collect data for audit purposes and participate in audit and protocol review as required

**Requirements**:

∙ Excellent telephony skills and phone manner

∙ Understand and observe strict confidentiality

∙ Undergraduate degree at 2:2 level or above

∙ Postgraduate Diploma/Master in Physician Associate from an accredited University

∙ Current and valid certification or recertification by the Physician Associate National Examination

∙ Member of Physician Associate Managed Voluntary Register
