The integrated Complex Assessment team are responsible for assessing and tracking patients following discharge from hospital that need further rehabilitation or assessment in a non acute setting.
The post holder will based within private Nursing home where MPFT have commissioned discharge to assess beds and will provide the discharge facilitator function for those service users.
The Post holder will work closely with multi-disciplinary teams under the direction of the Patient Flow Team. Gathering and recording information to enable accurate and individualised patient assessments to facilitate timely discharge of patients to their own home or an alternative care facility. The post holder will be responsible for the clerical coordination of the discharge process from the point of admission until discharge whilst building robust relationships with patient’s families and the multi-disciplinary team.

The post holder will be required to effectively provide support to assist in the delivery of safe and effective patient transfer and tracking within Discharge to Assess Services whilst gathering and collating collateral histories and information regarding current care needs.
The Discharge Facilitator will attend Multi-Disciplinary meetings, sharing and recording information to enable accurate assessment and robust flow through the D2A beds escalating any blocks or issues to the Flow Co Coordinator.
Effectively track the patients journey from admission to discharge in a timely manner using in house IT system CDT
Liaise with families and professionals including external agencies to provide timely updates and discharges including the booking of transport.
The post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives whilst working under the supervision of the Flow Team. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence.
The role will involve close working with health and social care staff and an understanding of community Home First services alongside knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A) would be desirable
