**Patient Services Co-ordinator**
To co-ordinate with the relevant GP Lead, all clinical recall systems run by the practice, issuing appropriate recall letters to patients at the relevant times and to supervise and report on these, and to monitor the outcome in relation to QOF targets for any given year.
To devise and implement new recalls as identified from time to time by the practice, and monitor the outcomes against QOF targets.
To co-ordinate the booking of reviews from set recalls e.g. mental health, asthma, dementia, rheumatoid arthritis, carer health checks, NHS health checks, heart failure, COPD, pre diabetes, diabetes, frailty assessments, blood tests.
To co-ordinate the childhood immunisation program to ensure that NHS targets for immunisation are met and that appointments and follow ups are monitored. To include any immunisation ‘catch up’ programmes as dictated by Public Health
To supervise the Local Enhanced Service for the Tracker Scheme for the Management of Vulnerable Adults at Risk of Admission, with particular attention to the review and audit of care plans/admission avoidance.
To co-ordinate and supervise the annual influenza vaccination campaign, shingles, MenACWY and pneumo campaign identifying at-risk group patients for call-in letters, agreeing clinic dates and times with the nursing team, supervision of vaccine stock, collation of numbers vaccinated.
To process and submit monthly Prescription Pricing Authority data in a timely fashion to ensure that payment to the practice is not delayed.
To review daily hospital admission data as received electronically and review this for submission to the bi-monthly clinical team meeting.
To maintain a register of Learning Disability patients who are resident at Sturts farm and the lantern community, ensuring each resident has a respect plan and on an annual recall for their LD healthchecks.
Online services - issuing login details and answering patient queries re their account. Setting up proxy access for care homes to order medication and monitoring proxy access.
Data entry - mammograms, diabetic eye screening results,
Reporting - e.g. whooping cough, implant, coils.
Maintaining the practices carer register, ensuring each unpaid carer is offered support from the surgery.
Medical records - summarising patient records on Systmone and liaising with Storofile for the collection of deducted patient records.
DNAR and respect forms - processing forms and forwarding to the appropriate agencies.
Use of Systmone clinical system - monitoring tasks, waiting lists and clinical reporting.
**Medical secretary**
- Referrals to both primary/secondary care including private referrals using electronic referral system.
- Medico-Legal work
- processing and invoicing medical reports, including PIP/ESA113/UC113 forms, insurance reports, solicitors’ reports, DVLA reports, DS1500 forms, adoption/fostering medicals, HGV/Taxi medicals, ‘To Whom it May Concern’ letters,
- Subject Access Requests
- processing SARs that come in, sending SARs to solicitors/ insurance companies etc.
- Safeguarding - child protection conference reports
The practice reserves the right to vary or amend the duties and responsibilities of the post holder at any time according to the needs of the practices business.
Able to be flexible in the working pattern.
