An exciting new opportunity has arisen to join the National Hospital for Neurology & Neurosurgery MND Care Centre team as their Care Coordinator. We welcome applicants from Nurses and Therapists for this Band 7 full time post for 4 years. The hours will be Mon-Friday with no weekends or bank holidays. You will join a supportive, forward thinking, and innovative multidisciplinary team who are committed to improving services and supporting patients from diagnosis of MND and throughout their illness.

This role will work closely with the Motor Neurone Disease Clinical Nurse Specialists, Consultant Nurses, Consultant Neurologists and the wider community MDTs and the Motor Neurone Disease Association

You must have an understanding of the needs of patients with a neurological life limiting illness; possess excellent communication skills for advising and supporting patients, relatives and other health professionals along with the willingness to learn about his specific area of neurology.

The band 7 care centre coordinator will have an appropriate professional qualification e.g RN or AHP registration. The post holder will;
- Practice autonomously and be self-directed, appropriately defining the boundaries of their practice whilst working in partnership with the MND team and the wider community based MDT.
- Coordinate the care of MND patients by liaising and attending borough specific MND team meetings monthly and making appropriate onward referrals to specialist teams as appropriate.
- Be responsible for updating and transferring information across to maintain the MND register
- Continue to develop own specialist practice and expert clinical knowledge base.
- The coordinator will help develop protocols, organisational guidelines and pathways of care. Ensuring NICE Guidelines in MND Management (NICE guideline 42, February 2016) and the National Service Framework for Long Term Conditions (NSF 2005) are followed. This includes following any revisions and updates to the National Guidelines
- Act as an internal and external clinical resource to other staff and students in specialist clinical area advising on evidence-based pathways and outcomes.
- Lead others to solve complex patient problems through an understanding of research, knowledge transfer, change management and clinical leadership processes.
- Build strong clinical networks.
