Job description

The core purpose of this role is to be a key worker for a young person within the Hub and to be a designated lead for a specialist area of responsibility. This job contributes towards achieving the aim of improving outcomes of children and young people within residential homes so that they can achieve the best possible outcomes in life. The post holder will be responsible for the direct day-to-day care of young people within a residential home accompanied by other supportive tasks and duties. Additionally, the post holder will also provide support to young people on the edge of care, accommodated in the various other hub placements, and care leavers (including post 18). The post holder will work within a defined culture and intervention practice model and will adopt a solution focussed and restorative approach.

The post holder will provide all aspects of care needs and interventions, in line with the young people’s Care Plans, and develop effective action plans with a focus on reducing risks and building on strengths. Key working is integral to the post; working ‘with’ young people to develop their resilience and life skills to enable them to successfully manage the transition to independence.

They will ensure that the support plans are being adhered to by all multi-agency professionals, to improve outcomes for children and adults.

Key characteristic requirements of this post are: flexibility, commitment, building effective relationships, resilience, persistence and the ability to progress plans.

Aspire Senior Hub Practitioners will drive the action planning, interventions and case progress for young people through effective communication with a range of professionals working together to improve outcomes for young people.

You must have an honours degree in a relevant field or equivalent experience plus a level 3 qualification (either NVQ or QCF Diploma in Health and Social Care) and have experience of first line management as you will be supervising a team of hub practitioners. You will have experience of maintaining financial recording and budgeting.

As a Senior Hub practitioner, you will be required to work flexibly to meet the needs of the service including working evening, weekends and Bank Holidays this includes sleeping in for which enhanced additional payments will be made.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.
