The Case for Further Change strategy references the need for further integration across health and social care organisational and service boundaries in Thurrock. Lack of partnership working can cause duplication of visits and tasks, handovers and referrals, the use of numerous systems and a failure to find out what matters to people means addressing or focusing on the wrong things. Lack of integration adds cost and demand to the existing system resulting in significant levels of ‘failure’ demand.

We want to significantly increase the functions of existing Wellbeing Teams to include tasks historically undertaken by health professionals to create truly holistic and self-directed ‘Health and Care Wellbeing Teams’. The Wellbeing Team have been chosen to pilot this project, due to their unique method to delivering care in the home. Thurrock Wellbeing Teams were introduced in May 2019 to address challenges faced by the homecare sector and to provide a person-centred approach to domiciliary care. Wellbeing Teams are small neighbourhood teams inspired by Buurtzorg’s neighbourhood care approach. The Wellbeing Team model aimed to revolutionise domiciliary care in Thurrock, provide an alternative to traditional time and task delivery model of home care and prevent, reduce, and delay the need for additional health and social care services by focusing on residents’ wellbeing and what matters to the person.

The post holder will provide education to the patient, their families and Health & Wellbeing staff as appropriate.

Provide comprehensive assessment and support to learners including those on work-based programmes, supporting staff to develop and obtain basic nursing skills as identified within the service remit.

The post holder will support staff to develop their careers, promoting active learning via apprenticeships and training.

Work as part of a multi-disciplinary team, acting as a resource to provide nursing advice

Develop and maintain a collaborative approach to Mental Health and Physical Health training and develop the competencies for this experience.

Encourage joint working and actively work towards improving outcomes for patients via the process of education and experience.

Keep accurate data and provide accurate information as required.

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2,011 per annum pro rata).
