Physician Associate in the Learning Disabilities Community Forensic Team.
**Job role**: To work within the framework of a multi-disciplinary team to provide clinical assessment and management to adults with learning disabilities who are at risk of offending. To work within the framework of health facilitation. To co-ordinate a clinical service under the supervision of the Consultant Psychiatrist.
To manage and co-ordinate an active caseload under the supervision of the Consultant Psychiatrist.
To focus on individual health outcomes for people with learning disabilities and facilitate with the MDT patients’ access to mainstream health services.
To assess, plan, implement, evaluate and review programmes of care and ensure measurable outcomes are efficiently and effectively carried out.
To have a working knowledge of the Mental Health Act, Mental Capacity Act, Valuing People and Human Rights Act and a general awareness of other legislation that impacts on the lives of people with a Learning Disability, including the Transforming Care Agenda.
To participate in the completion of relevant assessments and risk assessments.
To work with the individual their carers and members of the multi-professional team to provide psychiatric input to the development, implementation and review of a positive behaviour plan, risk assessments and management plans.
To participate in meetings, case conferences and clinics, where appropriate.

Birmingham Community Healthcare NHS Foundation Trust provides high quality, accessible and responsive community and specialist services within Birmingham and the West Midlands. We deliver over 100 clinical services, out in people’s homes and in over 300 hospitals, health centres and clinics. We provide services for adults, children, people with learning disabilities, those with rehabilitation needs and also dental services.
The post holder will join a team of consultant psychiatrists for people with intellectual disability who work with integrated multidisciplinary teams to meet the specialist healthcare needs of the population of approximately 1.1 million.
To work with the individual their carers and members of the multi-professional team to provide psychiatric input to the development, implementation and review of a positive behaviour plan, risk assessments and management plans.
To act, where necessary, as an advocate for the person with a learning disability, bearing in mind the rights, needs and dignity of the individual and development of self-esteem.
To participate in meetings, case conferences and clinics, where appropriate.
To work with the team to assess individuals in hospital and to plan for their safe and effective discharge in to the community.
To work with the team and the wider service to avoid hospital admissions by being responsive to crisis situations and the implementation of deterioration plans.
To keep up to date clinical records using the RiO system.
To liaise with all professionals, statutory and voluntary agencies working with people with learning disabilities e.g. Primary Health Care Teams, Clinical Justice Agencies, Social Care & Health, Education Services and others as required.
To liaise with all appropriate professionals, carers and the service user with regard to pre and post admission support to assessment/treatment/Acute Hospitals/Specialist hospitals.
