We are seeking a Mental Health Nurse, Social Worker or Occupational Therapy team leader to join our developing primary care Mental Health Team in the East of Southampton. You will be based within GP surgeries and working with service users of adult working age including older persons with mild to moderate mental health difficulties.
The team leader role is largely clinical; you will contribute to the triage service; offering early assessment, brief interventions and signposting for people with mental health needs in their local communities. You will play an active part in the development of systems and processes which support care delivery within these teams.
The successful applicant will become part of a growing primary care team who deliver care to some of the most diverse populations in Southampton. Our primary care setting is a rewarding place to work. We are looking for an enthusiastic and creative person who is committed to maximising life opportunities for our patients and their families. The PCN is a large, inclusive team with a focus on multi disciplinary working. There is an intention for mental health team to develop. The work is varied and challenging, and there is a strong team approach ensuring good support for the successful applicant. You will need to have a flexible, multi-disciplinary approach, have a good understanding of risk assessment, crisis planning and the recovery model.
