The aim of this role is to improve the health, care, and wellbeing of young adults in prison, predominantly within the age range of 18 - 25 years. This involves working collaborative with healthcare and prison colleagues to identify and manage the health and social care needs of each young adult and ensuring individualised, timely and appropriate access to services required by each young adult.

The Young Adult Health Lead will lead a team and manage their own case load of young adults within the prison. They will work with young adults and health care services and develop or redesign pathways of care to ensure that services are adequately responsive and able to meet the needs of young adults.

Engage YAs with complex needs who are not currently engaged in appropriate services.

Take responsibility for the daily management, coordination and delivery of the service, requires for the post holder to develop knowledge and skills appropriate to the service, and in embedding it into the fabric of the health and wellbeing of the YAs.

Identify the cohort and co-ordinate the development of person-centered support plan, developing integrated pathways into community to support individual health needs and aspirations.

Will be responsible for holding a caseload of approx. 50-60 YA aged between 18-25 years.
