You will have the opportunity to develop your skills while delivering support 1:1 to an individual in her own home. Undertaking a range of duties to support the client, you will work closely and collaborate with other support workers and professionals (Physiotherapist, Occupational Therapist, Psychologist, Case Manager) to deliver the highest standards of health care & rehabilitation to help her be as independent as possible.

You will have excellent communication and interpersonal skills and be passionate about your work, preferably backed by previous community experience and/or rehabilitation however full training will be given to those with the right approach and attitude. The post requires that you work independently to a very great extent and you must be reliable.

The post therefore requires that you work and conduct yourself in a manner that is acceptable to the employer. Reliability, integrity and complete trustworthiness are essential attributes in this post. Our client suffered a brain injury following a car accident in 2013, this left her with some cognitive and physical problems, she is friendly and personable.

She has a busy weekly routine, you will be expected to support her accessing local community activities and social events at the weekend, during the week she enjoys Pilates, Tai Chi, swimming, the cinema and shopping. Our client also enjoys holidays abroad and in the UK. Our client sometimes looks after the small, lively family dog which on occasions you will support the client to walk.

**Shifts**:
Support Workers required to work one weekend every 4 weeks on a rota.
- Shift 1. Friday 4.30pm to Saturday 10am
- Shift 2. Saturday 9.30am to Sunday 10am Shift
- 3. Sunday 9.30am to Monday 10.30am

**Tasks and Responsibilities**:

- Promote, monitor and maintain health, safety and security.
- Promote effective communication and relationships.
- Contribute to the protection of individuals from abuse.
- Enable the client to maintain her personal hygiene and appearance.
- Encourage the client to participate in recreation and leisure activities in the community.
- To establish and maintain a trusting, therapeutic, working relationship with the Client
- To be responsible for the client’s personal welfare
- To supervise activities in line with the support plan guidelines
- To contribute and support the client to complete household duties including, budgeting, meal planning, cooking, cleaning, shopping, washing and ironing
- To attend medical appointments with the client if required
- To liaise with other agencies when requested to do so by the Case Manager.
- To attend regular supervision with the Case Manager at intervals communicated to you by the Case Manager. These will be held on weekdays.
- To complete daily records as advised including; time sheets, mileage records, care plan summaries and critical incidents, petty cash records
- Attend review meetings with the employer and other persons as nominated by the employer
- To attend induction training at NCM (weekdays) including medication training, Acquired Brain Injury training and Basic Life Support plus complete online training within an agreed timeframe
- To recognise your own limitations and seek advice from the Case Manager as required.
- Any other reasonable task that is asked of you.

Northern Case Management (NCM) was established in 1997 to provide support to people with acquired neurological damage (i.e., brain and spinal cord injury) and empower them to develop their full potential and enrich their lifestyle through providing a comprehensive client focused service.

NCM work closely with the client and their family or representative to provide tailored support packages for clients in their own homes. The client or their deputy can be the employer or NCM can be the employer. The deputy is the employer for this client.
