Vascular/diabetic Foot Practitioner

The Vascular Service is looking for an experienced nurse practitioner or allied health professional to join the team as a Vascular/Diabetic Foot Practitioner on a full-time, permanent basis. You will deliver high-quality clinical care and excellent service to patients requiring vascular services and lead and support junior members of the team, both within the outpatient and inpatient environment as well as community teams. In addition, you will act as a primary contact for patients who have queries or concerns regarding the service.
You will work closely with the lead Consultants in both Vascular and Diabetes services and the Specialist Podiatrist, as part of the multidisciplinary diabetic foot team (MDFT). Alongside the Vascular team you will also assist with the development of the new hub and spoke model for our service.
The Vascular Service is a multi-disciplinary service within The Princess Alexandra Hospital and aims to deliver holistic care to all patients referred in both outpatient and inpatient environments.
**Your main duties will include**:
- Collaboration with the wider MDT, including Radiology, Theatres, Critical care, Outpatient staff, ward based nursing teams and community team
- Collaboration with hub and spoke hospitals, Leg Ulcer Specialist, Tissue Viability, Physio, therapy teams, Infection control teams and community teams.
- Acting as a main point of contact for service-related queries, and building a professional relationship with the patient and their family or carers.
- Assisting with the complaints process, using feedback to further develop the service and help to improve the patient journey
- Moving and handling of patients
- Frequent use of computers
- Frequent patient, carer and family meetings
- Frequent communication with other health and social care professionals which may involve travel to other clinics and hospital sites within West Essex and Hertfordshire
