Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services - and to prevent them becoming disenfranchised and isolated.
**Hours**: 28 hours per week
**Salary**: £21,507.75 per annum (pro rata)
**Holidays**: 25 days per year plus 8 public holidays
**Based at**:Porthmadog Resource Centre
**Job Purpose**:
The vision of this project is to achieve a holistic, innovative, and flexible service to support Services Users and Carers across Gwynedd to improve and maintain their health, well-being, and independence, through social integration.
Under the direction of their line manager, the post holder will undertake support planning, provide one-to-one, group support, activities, and interventions to improve the lives of service users and carers / families affected by serious mental illness.
To equip service users / carers and their families with skills, knowledge, and coping strategies to ensure their own well-being.
This is to be achieved by running sessions out of the resource centre and in the community, working in partnership with other statutory, voluntary, private sector organisations and community facilities.
