Care Co-ordinator

**About the Role**:
The Long Melford Practice has an exciting opportunity for a Care Co-ordinator to join our team.

The post holder will:

- Assist medical personnel in the care of practice patients, including treatment, preventative care, screening and patient education.
- Work with reference to practice agreed protocols and guidelines, providing Care coordinator treatments to patients both independently and in participation with nurse colleagues and general practitioners.
- Support the Long Melford Practice WGGL Primary Care Network (PCN) with their new responsibilities for improving health and providing additional care/services to their populations.
- Support the Long Melford Practice WGGL PCN with the provision of support and enhanced care to vulnerable patients and patients in residential and nursing home settings.
- Co-ordinate the work of healthcare professionals and non-clinical staff involved in the care of patients. Forming an essential part of a dynamic and forward-thinking Multi-Disciplinary Team (MDT) spanning PCNs, Community Services and Local Authority, working to provide enhanced care to these groups of patients.
- Assist GPs, Qualified Nurses and other practice/community/care home teams with the delivery of high quality direct and indirect patient care
- Provide assistance and support to patients, carers and the multi-disciplinary team
- Maintain high clinical standards
- Help maintain a clean and safe ward environment
- Role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams

You will have:

- Caring experience
- Understanding of patients basic needs
- Previous care experience preferably in a health care setting
- NVQ level III in health/Health& Social care/Direct Care or a portfolio of equivalent experience
- Completion of a course that demonstrate transferable skills
- Good verbal communication skills
- Good written communication skills
- Experience in providing direct and indirect patient care
- Good customer care skills
- Good General Secondary Education
- Good general IT skills

- The Long Melford Practice is a friendly, well organised and well supported PMS Dispensing Practice serving a population of 9, 500. The Practice is located on two sites situated in the historic wool towns of Long Melford and Lavenham.

We are a well-established, well-motivated Practice committed to providing high quality health care to our local population. As a GP training Practice we provide support to 1- 2 GP trainees and have the potential to support F1 & F2 medical students.
