We have an opportunity for a band 3 Healthy Lifestyles Support Worker to join our well established EI team. We work with individuals who are experiencing the first onset mental health problems, specifically Psychosis. The role of the EI MDT includes a detailed initial assessment which begins to inform the service user's care over the next 3 years. The team uses a Recovery philosophy to collaboratively identify the individual’s goals and needs and to support them towards achieving these. Many of our clients experience difficulties with housing, finances, vocational activities and social inclusion, healthy lifestyles and the successful applicant will be expected to work creatively, both with other team members and independently to support the client achieve their goals. We aim to support people to independence so they no longer need input from Mental Health Services.

The successful applicant will work alongside a dynamic, supportive, multi-disciplinary team of nurses, doctors, occupational therapists, psychologists and social workers as well other support workers/physical health support workers to deliver healthy lifestyle focussed interventions in the community. This includes a range of individual and group interventions. Your role will include working alongside the medical staff and other team members to ensure that our patients’ needs are met. The role is wide ranging, diverse and interesting, with no two days being the same.

The service will be fully integrated with local services, including Primary Care Networks, employment and housing services, VCSE local organisations and Social Support services.

**Main Responsibilities**:

- Supporting the physical health worker by carrying out physical health assessments.
- Promoting health lifestyles, for example exercise programs either via one to one support or groups
- Healthy nutrition advice, (losing, gaining or maintaining weight)
- Helping patients to access community resources to support a healthy lifestyle
- Creating relationships and links to other agencies / organisations or programs that promote healthy lifestyles

Personal qualities will need to include; experience in delivering healthy lifestyle support and interventions and an interest in promoting holistic health and well being, be positive and enthusiastic in what can be a very demanding role, the ability to structure your own work load and diary utilising a range of support tailored to the clients’ needs. Experience of using care notes is desirable and recording clinical notes, having strong IT skills is essential. You will also be expected to carry out support worker duties as per the job description.

We are looking for enthusiastic individuals. Good communication and problem solving skills are essential, knowledge/understanding of severe mental illness and the challenges this brings in daily life is desirable.

This role requires the successful applicant to be a car driver with access to a vehicle for business use. It also worth noting that this role will cover the whole of South Warwickshire.
