**ordinator**, supporting rough sleeping individuals throughout their treatment journey and successfully supporting their move on into suitable housing arrangements. We are hoping this role will be for you.

**This is an 8 month fixed term contract.**

**Responsibilities**:

**Hours**: 22.5

**Salary**: £26,393.77 - £30,316.74 dependent on experience

**About the role**:

- Establish effective communication and referral pathways those individuals who are homeless and in contact with other health and care agencies
- Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid
- Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working
- Liaise with other services involved in the care and support of an individual to ensure appropriate and secure transfer of all care needs
- Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and
provision of bridging prescription until an appointment and prescription is provided
- Increase and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting rough sleepers
- Ensure there are culturally appropriate interventions for hard to reach individuals from the Eastern European and Sub-Saharan African rough sleeping communities

**About you**:

- Have a strong understanding of substance misuse issues and experience of working within a similar field
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Have a good working knowledge of mental health interventions, services, and good practice
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
