An exciting opportunity has arisen for Band 3 HCA to join the Transplant Outpatients team.
This experience will allow the successful applicant to expand their existing skills and knowledge in this area of clinical practice.
The successful applicant will work alongside the transplant Nursing Team team supporting service delivery Monday to Friday in an outpatient settings.
Opportunities will be given for the development of advanced knowledge in caring for patients with Heart and Lung transplant, mechanical
circulatory devices and pre-assessment patients. This will give the successful applicant insight into the role of a Transplant Nurse in order to support the team and contribute to the patient pathway.

**Main tasks & responsibilities**:

- To carry out care and treatment, delegated from professionally qualified staff, requiring a wide range of technical ability, numeracy and

literacy skills as well as an excellent level of interpersonal skills.
- To supervise, where appropriate, less experienced Health Care Assistants in the clinical area ensuring patient focused care is provided.
- To provide any administrative and clerical support to the nursing team.
- To be flexible in the approach to the clinical area and provide cover for any areas specified (i.e. reception cover during leave periods)
- To ensure that the clinic environment is prepared each day for the patient journey to run smoothly.
- Under the guidance of a trained nurse to ensure notes are prepared for clinic and the appropriate investigations are ordered.
- To be aware of the national guidelines on booking appointments and giving patients choice.
- While covering Reception, to book follow-up appointments for patients attending the Transplant Outpatient Department in line with Trust Policy and protocols for clinics, whilst considering patient choice.

You will work with a team of Transplant Nurses who are responsible for the management of heart and Lung Transplant patients.
Care Assistants undertake a variety of interesting roles, which include phlebotomy and reception duties. Training will be provided.

- Patient/customer care (both direct and indirect)
- Carry out tasks to deliver and support direct patient care under the supervision of a registered nurse. Able to demonstrate abilities and competencies gained to enable tasks to be appropriately delegated.
- To assist with the scheduling of clinics by preparing notes with test results ready for patients to be seen by the medical/nursing team.
- Able to perform aspects of patient care, some with indirect supervision, eg, taking and recording of observations, MRSA screening, weight, height and BMI, blood glucose monitoring, collection of sputum and urine samples, simple dressings, removal of stitches, checking and preparing equipment and preparing patients for treatment. Refer to HCA duties document.
- Be proficient with phlebotomy skills as agreed, with the option to undertake intravenous cannulation training.
- Keep clinical areas clean and tidy and maintain a safe, pleasant environment.
- Adhere to departmental and Directorate procedures for the use of supplies and equipment in order to promote the effective and efficient use of resources.
- Assist in the restocking of supplies and maintenance of equipment as directed by a registered nurse. Also, to be responsible for maintaining a clean clinic environment, tidying/restocking of consultation rooms ensuring adequate supplies of appropriate equipment i.e. gloves, aprons, paper towels etc and providing support for all patients
