**Brilliant outcomes through integrated health, housing, and care. That’s when it hits home.**
We have a brilliant opportunity for you to join our transforming care service in Kent where you’ll deliver specialist Positive Behavioural Support (PBS) to our customers.
Working alongside our Registered Manager Pip and her team, you’ll be supporting our wonderful customers to live in their own homes across Kent. Our customers have varying levels of need with a huge amount of potential to develop their skills to be as enabled and independent as possible. For some customers these behaviours could include learning disabilities, mental health issues, autism and communication differences, alongside ongoing physical health conditions and some may also have a forensic background and display behaviours that challenge. They all need support and reassurance to help them make that exciting step to living independently.
As our PBS Practitioner, you’ll lead on functional behavioural assessments, PBS plans and interventions, working collaboratively with our customers and their families as well as clinical and operational colleagues. You’ll support the transition of customers into their new home, planning for their move and identifying and supporting the mitigation of any risks. You’ll use your experience and brilliant skills to ensure all practice is evidence based and person led, ensures safety and wellbeing, and maximises independence. You’ll love to share your PBS expertise, advising and coaching our colleagues on the day-to-day support required to support our customers.
**Be Yourself at Home**
Home Group is committed to tackling prejudice and discrimination, not just for our customers, but for our colleagues too We’re super proud of our internal Diversity Networks and allies (Multicultural, LGBTQIA+ and Disability) which support colleagues from different backgrounds to be their true selves. We recognise and celebrate our differences, together we make Home Group a great place to work

We are successfully building up teams across Kent for our customers. With services in Gravesend, Broadstairs, Snodland, Rainham, Ashington and Canterbury, you will be working with our customers as they transition and move into their own homes, as well as supporting them in the community.
**A bit about you**
You’ll be an experienced PBS specialist, with a relevant professional qualification such as Level 5 Registered Manager or a Degree in Learning Disability, Mental Health, autism or other health and social care.
