**Job Details**

We have an exciting opportunity to join the Fife Health & Social Care Partnership (Fife HSCP) as a Link Worker (The Well). The Well sits within Locality Planning Team, Community Led Support.

The post holder will be required to provide high-quality information and support to people who engage with The Well. The post holder will engage with people to provide information, support and guidance while delivering a community led support service by the Health and Social Care Partnership. Promoting and highlighting the work of The Well service, working collaboratively with the public, Fife Council, NHS and third sector organisations.

The post holder will provide effective administrative support services, which include preparing and distributing reports and managing databases/case management system.

**The Person**

The skills you will need:
Developing and delivering an integrated holistic needs approach (The Good Conversation) to promote early, targeted, planned intervention and support for the people of Fife in regard to health and social care enquiries.

Provide one to one support and advice to people using the Good Conversation approach and active listening skills to enable the individual to identify their own strengths and the things that are going well for them. Support people to access alternatives to statutory services and to divert away from these by exploring more creative and personalised solutions for the individual.

Supporting The Wells Coordinator in the delivery of the service, by engaging in various services across partners, attending meetings and engaging in various working groups.

Involving local communities in developing community led solutions for the population of Fife, including carers. Working across organisational boundaries with partners in Fife to ensure the best possible outcomes for people living in their local community.

Maintain and develop an effective reputation and presence for providing information and support to people who engage with the Well, including professionals. Maintaining information and monitoring systems relating to the post.
