**Most of our teams are agile working with the flexibility in working location. We currently have roles available in the following teams**:
**37 Hours Per Week Permanent - Health Transfers -** Social work team assisting people who are in hospital due to their physical health. Short term team. Has initial conversations to give advice or arrange hospital discharge support. Also works with people who are being supported by the reablement teams

**37 Hours Per Week Permanent - Adult mental health** - social work team supporting adults below the age of 65 whose primary needs are with their mental health and who are involved with professionals from Leicestershire Partnership Trust. Supports with discharges from mental health wards. Takes direct referrals from mental health professionals. Includes the approved mental health professional team who assess people’s safety under the Mental Health Act and the deprivation of liberty safeguarding team who assess people whose liberty may be deprived.

**37 Hours Per Week Fixed Term for 6 months (funding) -** **Learning disabilities and transitions - Social work team for adults with Learning Disabilities. The Transitions team can include working with some young people aged 16 and above to support with preparing for Adulthood. Accepts direct referrals into the service.

**What you'll be doing**

No two days are the same, you’ll be working with people who come into contact with us to have conversations to discuss what matters to them and others around them. You’ll have a can do attitude, a good awareness of the local information and advice that is available and use this to support people and their carers to solutions, drawing on what they already have around them that is working, or could work. You’ll work in partnership with others across social care, Health, Probation, and Housing teams.

You’ll have experience of working with adults drawing on care and support. You’ll demonstrate an interest in people and will be trusting, honest, kind and transparent in your daily communication. You’ll enjoy working in a busy, fast paced environment, thrive under pressure and be energetic and efficient.
