**The Role**:
With an overall focus on improving outcomes for vulnerable children, young people and their families, this is an exceptional opportunity to help families ‘navigate’ our services so that they can work towards a better future through carefully-considered interventions. This is for a project looking at health inequalities amongst children under five.

Our Family Hubs are great source of information, support and advice. As part of the targeted Early Help system, they provide the means with which families can start to make a change for the better. In this newly created role, you will be the first point of contact for families coming into the Hubs, so you will have the scope to get vulnerable people off to a great start. Acting as the bridge between families and our multi-agency services, you will work collaboratively with them to bring together the factors and the support that will lead to lasting positive change.

Managing a caseload of families who have varied needs, you will be involved in every aspect of their journey with us, ranging from initially assessing their situation and helping them access the services that are best suited to them, through to making referrals and flagging up child safeguarding concerns. Seeing to it that families and young people remain involved in all that you are doing on their behalf, you will work with them in a number of settings to help them understand our processes and how we can support their progress.

You will also help us raise awareness of the value that can be gained from visiting our Hubs. This will include working with GP's and schools to identify families who would benefit from our support.

**About You**:
This is a fantastic opportunity to make a tangible difference to some of Westminster's most vulnerable families and young people. To be equal to the role, you will therefore have experience in family support work either in a paid or a voluntary capacity. With a good awareness of the issues affecting families in inner-city communities, you will be able to build trusted relationships with those in challenging circumstances. This will include an understanding of the issues faced by young people who have special educational needs or disabilities, and you will need to have a good understanding of child development and wellbeing. You will also have an awareness of the Early Years Foundation Stage and Early Years Outcomes.

Maintaining accurate notes of every interaction with the families and the measures that you have put in place, you will need excellent IT and verbal and written communication skills, with the capacity to create insightful reports. Bringing a sharp eye for detail to every interaction, you will work compassionately, and you will be capable of quickly building relationships with Hub users. This will include reading their voice and body language to get a full understanding of how they're feeling, and better determine how we can support them.
