To promote the philosophy of person-centred care, establish and maintain a supportive relationship with people affected by lymphoedema.
To work as an autonomous practitioner and has the freedom to act to influence patient care and service provision.

To work within the specialist cancer site-specific Multi-disciplinary Team (MDT) within the Betsi Cadwaladr University Health Board, be a core member of the MDT and the designated named lead nurse within the team for those patients.

To be responsible for the management, delivery and on-going future
development of a high quality, accessible nurse led service.
To put quality at the heart of practice by delivering evidence based,
individualised and personalised care through holistic needs assessment,
planning and evaluation of care and all care interventions.

1. To demonstrate advanced and highly specialist practice in holistic
assessment, planning, implementation and evaluation of specialist practice
to patients and their families, according to changing health care needs.
This will necessitate high levels of autonomy and advocacy and include
receiving direct referrals, undertaking differential diagnosis and ensuring
the delivery of ongoing appropriate care, to maintain quality of life.
2. To define a clear care pathway, referring to other professionals and
sign-posting to other agencies as appropriate
3. To manage a specialist caseload, and be responsible and accountable for
delivering evidence based individualised and personalised care through
assessing, planning and evaluation of all care and interventions.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
