Come and join our innovative team in Hwb Iechyd Eifionydd. The surgery is moving towards a MDT model, which has provided an opportunity for an additional Advanced Clinical Practitioner to join the team providing patients with the opportunity to see the most appropriate practitioner to effectively deal with their presenting condition at the first point of contact.

This is an exciting opportunity to join the friendly GP practice team as an Advanced Clinical Practitioner for the Health Board Practices at Hwb Iechyd Eifionydd. We offer a Multi-disciplinary Team model, providing patients with the opportunity to see the most appropriate practitioner to effectively deal with their presenting condition at the first point of contact.

The extended GP practice team consists of Salaried GPs, Practice Nurses, Health Care Assistants, Practice Pharmacist, Advanced Physiotherapist and voluntary sector. The environment is of a happy and supportive nature with the team looking after each other and is always looking at innovative ways of working. Clinical and peer support is available.

We are looking for a person with a proven record/special interest in caring for patients with minor illness and some experience of dealing with Chronic Disease Management.

Applications will be considered from individuals interested in part time or full time hours. Work base to be discussed further at informal visit or during interview, this post will also require the successful applicant to visit patients in their own homes.

There may be a requirement for you to work occasionally at Canolfan Goffa Ffestiniog, Blaenau Ffestiniog.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
